Patient Participation Group

The Church Street Practice Patient Participation Group (PPG) is a collaborative partnership between the practice and its patients. We aim to represent the patient community, support the practice, and work together to improve services and patient experience. We engage in regular discussions, provide feedback, and support health initiatives within our local community.

Church Street Practice PPG Meeting Minutes

Practice Report

  • Total Triage: Successfully implemented, benefiting both patients and staff.
  • On-line Registration: Open to new patients only, no transfers from NS. CS has 19,000 patients (NS has ~16,000), and closing the list is a possibility.
  • Staffing: 2 new GPs to start in August, reducing locum budget. New PA hired to replace Kizzie’s role.
  • Website Update: CS is working on a new website layout, similar to NS and the NHS App. Digital funding is being sought by the PCN Digital Lead.
    Action: JB to send information to AD for newsletter inclusion.

Digital Sub-Group

  • Next Digital Café is on Friday, 26th July at the Mably Way reception. Sessions are held on the last Friday of every month, with 20-minute timed appointments and 5 helpers available.
    Action: PPG members to volunteer for sessions.

Reports

  • Jaz Kundi (Healthwatch): Congratulated CS PPG for their active involvement. Encouraged feedback from patients who have recently left the hospital.
    Action: PG to include details in the next Joint Newsletter.
  • Ian Hiscock (Stop for Life/Health Checks): Provided a report on smoking cessation. [See Appendix A.]

Joint Newsletter

  • Items agreed upon: Exclude NS PPG mission statement, omit previously used diagrams, and keep content concise and patient-focused.
  • Printing Deadline: 19th July. Next edition planned for October.
    Action: AD to finalize content.
  • Distribution List: JC created two versions (Excel and Word). Finalize data for poster and list.
    Action: JC to finalize QR coded posters before the 19th July.
  • QR Codes: Discussion on whether they need to be static or updated for each newsletter.
    Action: GT to email PW.

Notice Board

  • Rearrange to create more space, use A5-sized newsletters, and add an email link for PPG minutes.
    Action: JC

Finance Report

  • Current Balance: £218. Sponsorship funds expected for Health & Wellbeing Event (April 26th, 2025) from various organizations.

Outreach Work

  • Continues to revisit retirement homes. Difficulties accessing council-run homes noted.
    Action: STh

AGM – 12th September 2024

  • Pre-AGM talk open to both practices, covering topics like Total Triage, Blood Cancer Awareness, and NHS App/Digital Café.
    Action: GT & JB

Health & Wellbeing Event – 26th April 2025

  • Funding: £500 each from Assura and Grove Council. Further sponsorships being pursued. Subgroup to meet on 22nd August via Zoom.
