Routine Appointments
To book a routine appointment with your GP, use our patient triage service.
You do not need your NHS login details to use patient triage, but it will make it quicker to use the service.
To use patient triage, you will be asked the following:
What would you like help with?
- Admin Query
- please use this form for when you do not need medical attention or advice
- New or extended Fit (sick) notes
- Repeat prescriptions
- Letter or form requests (this is a chargable service)
- Copies of test results and hospital letters (also available from the NHS App)
- To request a blood test appointment (you will be sent a booking link to your mobile)
- To request a routine follow up with a GP or pharmacist
- To request a physio appointment
- Medical Request
- If you need to speak to or see someone about a new or ongoing problem
- Your request will be looked at by a clinician within a couple of hours and you will be directed to the most suitable service for your issue. This could be:
- A same day telephone or face to face appointment
- A routine telephone or face to face appointment within the next two weeks
- A blood test or physio appointment
- A referral to community services
- Advice on how to manage your symptoms at home
Emergencies
Patient Triage is not an emergency service. If you have an urgent problem you should call the practice on 01235 770245.
We have an emergency phone line through our phone menu. After calling, choose option 1.
Our Duty GP can then be asked to speak to you straight away and deal with the emergency appropriately. Our emergency phone line is open from 8am to 6.30pm
If you have an emergency outside of these hours, you should call 111.
Home Visits
Home visits should only be requested for those who are unable to come to the surgery because of serious illness. Visits should be requested before 10.30am if possible.
Before the doctor or Emergency Care Practitioner visits they may phone you to ask further details of the problem. This helps them make sure that they are bringing the right equipment with them or whether the problem may be dealt with in an alternative way.
If you’re unable to attend an appointment
If you are unable to attend your appointment you should contact the practice as soon as possible so we can offer the appointment to someone else. We have a text reminder service which will text you the day before your appointment is due.
