How to get your GP record

Your GP record includes information like any conditions or allergies you have and any medicine you’re taking.

Most patients will automatically be given access to more information added to their GP record from November 2023 onwards. This includes letters, test results and appointment notes.

Some people may also have access to information added to their GP record before this date.

You can access your GP record and nominate someone you trust to access it too.

There are 3 main ways to get your GP record.

Using your NHS account

You can get your GP record by logging into your account using the NHS app or NHS website.

First, you need to register for online services and prove who you are. You can do this when you create an account.

You’ll need to ask us for online access to see details of consultations before November 1st 2023. you can do this by emailing bobicb-ox.churchstreetpractice@nhs.net 

Using other online services or apps

You might be able to use other GP online services and apps to get your GP record.

First, you need to register for online services and prove who you are.

You’ll need to ask us for online access to your full record, or you’ll only see your medicines and allergies. If you would like to sign up for a service that is not the NHS App please ask a Patient Adviser for a form or download it here.

Can I access someone else’s medical records (health records)?

Health and care records are confidential so you can only access someone else’s records if you’re authorised to do so.

To access someone else’s health records, you must:

  • be acting on their behalf with their consent, or
  • have legal authority to make decisions on their behalf (power of attorney), or
  • have another legal basis for access

Accessing children’s records

A person with parental responsibility will usually be entitled to access the records of a child who is aged 11 or younger.

Children aged 12 or older are usually considered to have the capacity to give or refuse consent to parents requesting access to their health records, unless there is a reason to suggest otherwise.
